Caregiver Self-Checks

Everyone like to check off their “to do” list! The PC TALK Self-Checks can be used to plan strategy use in the coming week, and caregivers can check off the PC TALK strategies used during daily routines.

Before using the Self-Checks, it’s recommended to review both pages with the caregiver. The Planner can facilitate discussions with caregivers to help determine what strategies and routines the family will focus on throughout a week. Responses from the Planner can then be used to mark the strategies and routines on the Caregiver Self-Check.
